What ARE Those Little White Bumps?

One of the most frequent concerns expressed on our WebMD GYN message board, and our "Friends Talking" message boards relates to "bumps" or "pimples" on the vulva. Usually these lesions have just been noticed, rather than being present for a long time. So let's consider what some of the causes might be.

Occasionally a woman will refer to the raised lesion as a "cyst". Cysts are a common occurrence, and can arise most any place on the body. In the area of the vulva a cyst is usually created by a blocked sweat ("apocrine") or skin ("sebaceous gland") gland. For example, if a hair follicle gets blocked it can fill with the debris of exfoliated/shed skin cells which causes the lining of the follicle to get distended. This is similar to what happens when one develops a "white head" pimple on the face. On the genitals, such cysts are called "epithelial inclusion cysts".

If the cyst continues to enlarge to a size greater than a half inch, it can incised with a sterile instrument and drained in a sterile fashion. While some women attempt to "squeeze" a cyst to extrude the "cheese-like" skin-cell debris, this is not a good idea. Normal skin bacteria can enter through the break in the skin and set up an infection.

There are some specific genital sites where drainage ducts can get blocked and create cysts. These are named after the area in which they occur:
  • Skene's duct cysts are located on either side of the urethra. These may be treated with warm moist soaks--or can require incision and drainage.

  • Vaginal cysts of embryonic origin typically are not a recent blocked duct opening. Rather they are a cyst which developed in utero. These are treated with surgical excision if they continue to grow in size over time.
  • Bartholin's duct cysts can develop on either side of the lower portion of the labia majora. These can grow to be the size of walnuts and usually are noticed because of a rapid increase in size and tenderness. Three times per day warm soaks or sitz baths are tried first, followed by incision and drainage if needed. Bartholin's cysts tend to reoccur. If this happens the cyst may be surgically opened and the sides sewn open (like keeping a purse permanently open) to be sure no fluid or skin cells accumulate. This procedure is called "marsupialization."
There are two conditions where clogged sweat glands can create bumps on the vulva:
  • Fox Fordyce is an itchy type of skin lesion which can range in size from small papules to cyst sized bumps. They may also be found on the lower abdomen and thighs. Keratin or skin debris clogs the apocrine/sweat glands, thus it is not a bacterial infection which requires antibiotics. It may be treated with a steroid cream or phototherapy.

  • Hiradenitis suppurativa, by contrast, is a clogged sweat gland which often has a secondary bacterial infection associated with the bumps. This condition does not itch, but can be very painful if deep, infected cysts or nodules form. These areas tend to leave a shiny scar of darkened skin. Somewhat more common in women who have acne, these lesions are often treated with acne-type approaches such as oral or topical antibiotics, or even oral Accutane. Steroids and birth control pills have also been tried. If there are large hardened nodules, or draining fistulas (tracts for pus to travel from one nodule to another), the area can be surgically treated

While on the subject of infections we should mention some of the most common vulvar infections which can create bumps or pimples:
  • Folliculitis is the most common cause of irritated or tender, raised lesions. Common skin bacterias such as staph or strep get into a hair follicle through a tiny break in the skin (eg bikini shave nicks). The area of infection can remain a small "folliculitis" or it can increase in size to become a "furuncle." A larger, or deeper, infected skin abscess is called a "carbuncle." Depending upon the size and degree of infection, incision/drainage or antibiotics may be utilized as treatments.

  • Skene's duct, or Bartholin's duct, abscesses might require antibiotics if a bacterial infection takes advantage of the cyst formed in the clitoral or labial area.

Sometimes the infection is viral rather than bacterial:
  • Molluscum contagiosum is caused by a pox virus. The bumps on the vulva or thighs are small, fleshy, and round often with a small indentation in the middle of the round lesion--like a bellybutton. These bumps tend to regress in time even without treatment. For cosmetic purposes they can be removed by freezing/cryo.

  • Human papilloma virus (HPV) subtypes 6 and 11 cause the visible genital warts which look like little cauliflowers. These rough to touch, raised growths can expand in size and number--especially in times of a suppressed immune system (eg pregnancy, illness, etc.). HPV lesions can be treated with applications of bi or trichlorocetic acid, freezing/cryo, or an immune system enhancing cream ("Aldara").

  • Herpes simplex virus (HSV) is more often linked to an itching, burning area which might be slightly raised. It can look like a bug bite in its early stages, but within several days it becomes an open ulcer or cut in the skin--not a bump.

There are a number of medical conditions which can produce bumps which are not related to clogged ducts or infections:
  • Acanthosis nigricans produces velvety, dark raised areas on the vulva, armpits, and neckline. It is linked with insulin resistance, the metabolic change found in polycystic ovarian syndrome ("PCOS").

  • Neurofibromatosis creates nodular areas all over the body, not just the vulva. This is an uncommon, genetically transmitted disorder.

  • Angiokeratomas as the name implies are created by small blood vessels. When there are dilated capillaries grouped together dark red to purple raised spots are visible. There may not be any symptoms until friction to the capillaries causes bleeding. Treatment is by freezing/cryo or surgical removal.


Lastly there are "normal" findings which a woman may suddenly discover:
  • Skin tags, or even remnants of the hymenal ring around the vaginal opening, can be mistaken for bumps. As one might expect these are soft and fleshy with irregular shapes.

  • Papillomatosis is frequently mistaken for genital warts, but is a normal variation in the vaginal/vulvar anatomy. If genital warts look like a cauliflower, these look like stalks of asparagus--more fingerlike than raised bumps.

Given the multitude of possible causes of bumps or lesions in the genital area it is important to have them examined by a GYN or other healthcare provider to ensure proper treatment. For more in-depth information you can visit this WebMD site targeted to healthcare personnel.

eladii- go to the doctor!!!! you need to see a gyno, especially since you are pregnant. Or if you can't afford that, Planned Parenthood.

porkey99- you need to go to the gyno too but because you have herpes. And if it isn't herpes then you may just have "pimples" down there, but it seems more like herpes. =/ Sorry

okay so about a month ago after having sex with my boyfriend i discovored a bump right behind the hole of the vagina. it had no color, it didnt hurt so i just figured it was a scab from havin too much sex. but after that i just kept getting more and more.they moved up the right side into my lips and now are on the left side.and theres one big one right on the hole. they are white now and i have no idea what they are. my boyfriend doesnt have these so i need help to figure out what they are and how to get rid of them.

Hi, I am currently 16 years old. I have noticed little white bumps on my labia and around the skin down there. It cant be an sti because Im a virgin. I first noticed them when I was around 12, when I got my period. They are still there, and I had forgotten abou them. They arent painful, and they dont itch. I dont have abnormal discharge. What is it? They are just tiny bumps that are white and pin-head like. How to get rid of them? Am I abnormal? Thanks.

I went to my doctor a couple of weeks ago and had them look at a couple of bumps. They put me on aldara and since then the whole inside of my vagina lips are red, burning, itching and I have sores everywhere. They are very painful. Is this part of the medication or is there anything I can do? I am just so uncomfortable and in pain.

Wondergirl....these are normal as long as they do not get inflamed or hurt. I, too, have had them my whole life. They are a form of milia. They won't go away on their own and any attempt to have them removed by a doctor will not be worth the trouble. Many women have these. You are not alone. It's just the make up of your body. No worries and nothing to be ashamed of. Enjoy your body and get to know it. You will find this knowledge will benefit you in the long run. Take Care.

ok, i just got off of depo and i just found this bump next to my hole to my vagina about a month ago and it hurts and it looks like a zit and all that but its not.. its about an inch big and hard like if theres a bullet inside... i tried poppin it and all i got was blood nothing else... i know its not a std bc i got tested about 2 weeks ago for it... and now lil bumps are everywhere down there about 1/8 inch big and the are all itchy and im really red and dry and its hurtin me wen im havin sex... is this common or am i in bad trouble?

i have these little lines on the skin really close to the vagina opening all around that are really itchy. ive tried to find out what they are but theres not much said about it. i dont think they are serious, i dont have pain just itching. they are really really small thin lines about 1/4" long...

oops sorry well hi i am 18 yrs old and i've never cheked myself in my vagina with attention before like now and i recently felt a bump like thing in my vagina on the left side and i was wondering if i should be conserned, it doesnt hurt and it doesnt itch it doesnt realy bother me while having sex but im not sure if i had it when i was having sex it's been like 3 months since my last intercourse what should i do?

Ok hi, Im not really sure when, or how long ive had whatever i have, but last year 07', i noticed a few white bumps clustered together on the inside on my vluva. I never really paid any attention to it because i didnt really think it was anything. But now I look at it, and it looks to me like they have gotten a little bit bigger. Ive done my research, but im still not sure what it is. Help?

Dear Alice,
I two am a Diabetic and have been for the past 12years. I have had the same small white bumps for as longs as I can remember. I wouldnt be concerned as long as they do not get inflamed or hurt. I do know that when my sugar has been high for a long period of time or my A1C is over a 7 that I get yest infections more often as well. I hope that this helps you and take care.

Hi, I'm so worried and I'd really appreciate some advice. I noticed a small flesh coloured lump on the left side of my outer labia. It's since gotten bigger but is still quite small. It's been there for a few months. I also noticed a lump on the inside of my outer labia (if you get what I mean) on the right side, and I'm so so worried. I'm in a monogamous relationship for 2 years, and my boyfriend has no symptoms. I have been on a combined pill for one year. Please tell me I'm normal, I'm so so stressed and worried and would appreciate help?

About 4 months ago i noticed these little reddish white looking bumps, some flesh color, on my right buttocks and inner thigh close to my vagina, they dont itch, burn, or have anything coming out of them, when I squeeze them they just bleed. I went to see my OBGYN and he said it didnt look like an STD....but to give it time and see if they go away on their own...I'm really confused now...can anyone help me or should I see a skin doctor??

That was really helpful, thank you. I've been worried because I felt something hard above and to one side of my clitoris, and decided to take a look at it today with a mirror. It is a pure white bump. I'm relieved to know it could be one of those insignificant things you mentioned. I can't get medical care, because I'm an uninsured American without multi-millionaire status, and am looking forward to Obama's changes in the system.

im almost almost 20, female, and a virgin, no one has "gone down" on me and no penis has come close to my vag. the thing is i have always had some bumps around my vag, ive had them as long as i can remember, ive seen pics of herpes and it looks a lot alike, but since im not sexually active in anyway im confused, also i heard herpes comes in outbreaks....mine are always there...is it genital herpes or something else?

Hiradenitis suppurativa can be cared for with a $4.00 cleaning agent from the drug store!! I suffered with these boil-like bumps in my under arms and genital area since puberty. After showing them to all of my doctors for YEARS, I was finally introduced to Hibiclens by my dermatologist. Now I use Hibiclens every time I shower and the bumps occur every once in a while but are never serious. And, they don't scar anymore. I can't believe I suffered for so long, especially since the treatment is so easy and cheap. I hope this helps someone.

Aphthous ulcer, grossly obvious gingivitis, trauma, ulceration, erythema, desquamation, coated tongue, keratinization, geographic tongue, mucocele, and short frenum have been reported rarely with chlorhexidine oral rinses. Gastrointestinal side effects associated with chlorhexidine oral rinses without oral hygiene measures have included increased gingival bleeding after gentle massage.

Parotid gland swelling has been reported rarely.

Gastrointestinal side effects associated with chlorhexidine cleanser have included a single case report of acute gastritis.

Gastrointestinal side effects associated with chlorhexidine cleanser enema have included a single case report of acute colitis.

Gastrointestinal side effects associated with the periodontal chip have included transient toothache (50.7% vs 41.4% placebo), including dental, gingival, or oral pain, tenderness, aching, throbbing, soreness, discomfort, and sensitivity.

In clinical trials, 56% of chlorhexidine oral rinse users had a measurable increase in staining of teeth, and 15% experienced heavy staining. Stains may generally be removed by conventional professional prophylactic techniques. Staining of rough areas may be permanent. Stains are general dark brown to blackish.

Staining will be more pronounced in the presence of heavier accumulations of unremoved plaque.

Staining due to chlorhexidine may be due to the interaction with dietary compounds such as coffee and tea.



Hypersensitivity side effects
Hypersensitivity reactions to chlorhexidine have included allergic contact dermatitis, pruritus, vesicle formation, urticaria, dyspnea, and anaphylactic shock.

Hypersensitivity reactions have been reported after the use of products containing chlorhexidine as a preservative or devices coated with chlorhexidine, including allergic contact balanitis and anaphylactic shock.

Patch testing using chlorhexidine has revealed positive reactions in more than 2% of patients tested. In eczema patients, the rate may be as high as 5%.



Ocular side effects
Ocular side effects associated with chlorhexidine cleanser have included ocular toxicity.

Ocular side effects associated with chlorhexidine containing contact lens solutions have rarely included allergic conjunctivitis.

Exposure of the eye to chlorhexidine cleanser, generally during preparation for facial surgery, has resulted in eye pain, edema of the epithelium, keratitis, inflammation of the conjunctiva, corneal epithelial cell loss, chronic corneal ulcers, and opacification. Corneal transplantation to correct permanent damage has been required in some patients.

Allergic conjunctivitis is rarely associated with the use of contact lens solutions which contain low concentrations of chlorhexidine as a preservative.



Nervous system side effects
Nervous system side effects have included cases of sensorineural deafness following direct instillation of chlorhexidine into the middle ear.

Nervous system side effects associated with insertion of the periodontal chip have included headache (27.1% vs 27.5% with placebo chip).

I am 18 years old and I have never had sex, never been on birth control and yet I have a large red bump on my left outer labia. It is only painful when I wear thongs or sit with my legs crossed and it's not terribly painful just like a dull pain. Someone please help and tell me how to fix this!

I have a question. I recently had an abortion(a week ago) and I ended up having unprotected sex last night. If I take the Plan B pills, will it work the same as if I hadnt had an abortion, or will that affect the way the pills work?

All that and I forgot to post my problem. Anyways I recently have started noticing small flesh coloured bumps on my labia, they are not very itchy, they arent really red, they arent painful, and there is usually only 1 or 2 maybe and sometimes I dont have any. Im wondering what this could be because it doesnt seem to cause any problems for me Im just curious what it could be becase it hasent really happened before and Ive had the same sexual partner for 8 months now and its only happened maybe within the last 2 months maximum. Any suggestions or help would be great

alrighty so im 15 and i have 2 or 3 little bumbs on the clit and i just noticed them last night but i have no idea when they got there and idk what they are. im not a virgin, so i dont know if it is an std or if it is a pimple?? but i have a bf and i doubt he has an std, but i never asked. what should i do?

So, I'm a virgin and I noticed two bumps on the lips of the vagina.. I noticed if i squeezed them, they'd pop like a zit and had a white/reddish drainage. I don't know if i should pop them but it felt like a zit. I looked at it and it's like a bump.

Any suggestion what it might be?

I recently noticed a bump on my clitoral area and it seems to be very tender and soar. I noticed it maybe a day or two after I shaved. It looked like a whitehead last night and today when I checked to see if it was getting better, it was more the color of my skin. This is my first time having something like this and I wanted to know since it was soar and tender should I be concerned or just wait for it to go away? Is this normal? Could it be because I shaved?

hi im 16 years old and i have had little raised bumps that scar after a while if messed with and also bumps from shaving irritaion around my vagina for a very long time. even before i was sexually active. theyve bin there since i was about 11 or 12. rite around the time i started puberty. i also get the same type of bumps and symptoms under my armpits. ive bin so worried ive tried everything from acne washes and cleanser, bath soaks, only wearing white cotton panties and certain clothes but nothing seems to work. i would really appreciate some advice. ive talked to my mother about it but shes not very open to this type of topic and im too shy to ask any friends about it. id go to the doctor but unfortunatly i dont have healthcare or any type of insurance. help porfavor!!

Hello. I am a 19 year old female who just started noticing a lot of tiny, pinhead like size white bumps on the inner lips of my vagina where the smooth surface meets the hairy surface and above the clitoral hood. I haven't tried, but it appears as if they could be popped like whiteheads. They are never sore, irritated, or itchy in any way. I have only been sexually active with my fiance in the past 4 years and had 1 sexual partner before that. I have had yearly paps since age 15 and they have all came back normal. I even asked my fiance if he had noticed anything unusual or if there was something I should be concerned with and he said no. My gyno can't get me in for another 2 months and I am just looking for some relief until then. I want to say they have been there since puberty but I can't remember that far back.

Most of the little bumps are TOTALLY normal... they are most commonly known as Fordyce spots. Google the term for more information. (I visited my gynecologist and that is what she told me they were. They are completely safe and normal and will not cause you any harm! They do not go away, but may fade over time. Removeall is possible, but also pointless.

Blogger Eladii posted something on March 13, 2008 that is EXACTLY what I got about 6 days ago. The only difference is she was pregnant and I am not. I have not had any thing wrong down there or changed my partner recently. He is fixed so there is not a chance of me conceiving. (We already double checked) Anyway, I have finger like growths that are flesh color and some have white tips. They don't hurt, burn, or have any odor. They're just there on my inner labia. Does anyone know of a place that has information about these symptoms?

I have a very small, white bump on my labia (very close to my clitoris) that I just noticed in the shower this morning (it looks very much like a pimple). I've never noticed any bumps like this on my vagina before, and I just had a pap smear in June (2 months ago) which came back totally normal.
I'm in a commited relationship, but I'm worried because my boyfriend had a previous partner with herpes, and he swears he's clean, but this bump raises serious concern for me.
The bump does not hurt or itch, but since it's on the inside of my labia, I don't think it can be a clogged hair folicule.
Should I be worried, or is this normal?

Ive had the same problem as Wondergirl.. Ive had these white bumps ever since i had my first period when i was 12.. they dont hurt.. i forget about them to be honest..they're just kinda there.. It use to freak me out, I thought something was wrong with me..I also have a skin condition where I have a continous rash that is unnoticable.. i wonder if they are associated with one another? anyways Im glad to know im not alone.

Ok so about a week ago I got a bunch of blisters on my skin next to my vagina. A few days later they popped and now its one sore. My fiance gets cold sores but we never have oral sex if he has one, but can I get genital herpes that way? Is it herpes or something else.

I've recently shaved my vagina and my area now is swollen and has these little puss like bumps and it itches. I've never experienced anything like this before and it's starting to worry me. Could it maybe be razor burn? I am sexuallly active and I've been pertecting myself. Any ideas what it is and should I b worried?

I've had two bumps on my inner lip. I had the first for about a year and finally got frustrated and squeezed it. The second I squeezed the day I noticed it. Both times, a hard white blob popped right out, it bled a little, then healed up. Any ideas?!
